Abstract:
Introduction: In patients with OSA , there is increased day time sleepiness due to recurrent cortical arousal at night time which are secondary to disease. Patient is morbidly obese with BMI>30 and increased neck circumference.
Presentation:
- Patient feels hypoxic due to flow limitation which causes cortical arousals.
- Loud snoring causes narrowing of airway which lead to an apenic episode.
Methods:
1. Polysomnography
2. Echography to check LVF
3. EEG EPWORTH SLEEPINESS QUESTIONNAIRE FOR SLEEP STUDY
Results:
Apenic episode results in grunting at the end which helps to forceful opening of mouth and allows passage for air to enter. Each episode of apnea represents a reduction in breathing for at least 10 s and commonly results in a ≥3% drop in oxygen saturation.
Conclusion: This sudden air passage causes rise in SpO2 and increase in heart rate , which suddenly increase heart load resulting in Left ventricular hypertrophy or left ventricular strain.
Objective: to study how obstructive sleep apnea increases CV mortality Obstructive sleep apnea (OSA) is present in up to 15% of middle-aged adults and has a strong association with obesity.
Key words: Cortical arousal , apneic episode and left ventricular hypertrophy
